If this is your first visit to our office, please click the patient portal button below to register using the directions below.

  1. To book an appointment – You can access our online scheduler without registering, but registration is required in order to access the online entrance forms. When you register to book an appointment, your browser will ask for access to your location, otherwise please enter Calgary as your location, and then select our office. You can then proceed to book your appointment and we will email you once accepted.
  2. Once you have received email confirmation of your appointment, please log into the portal and select the Forms option from the left margin. Please complete BOTH forms. You have now completed most of what we would have needed in the office!

After this, please read the information below this box and view or download any applicable forms. If you would like us to make an online claim for insurance, please print out and sign the forms below to bring to your appointment. We know this is a lot of work but it will make your appointment much more efficient!

Your portal can now be used to check all upcoming and past appointments, your order status, and to reprint any receipts for your records. It also allows secure messaging with our office. We are committed to protecting your privacy and compliant with HIA and PIPA legislation.

Please bring your contact lens boxes/packaging and all eyeglasses and prescription sunglasses to your visit. New patients for a comprehensive exam should allow at least 1 hour for the appointment. This gives you sufficient time for getting any required imaging, contact lens assessment and/or choosing eyewear. Directions and parking information can be found here.

Children and Seniors eye exams are fully covered by Alberta Health Care. Coverage of up to $230 towards eyewear is also available for seniors on limited income. Coverage for all ages is provided by Alberta Health for medically necessary visits such as eye infections, diabetic retinal exams, glaucoma testing and select others.

Routine examinations and eyeglass prescriptions are not covered by Alberta Health for ages 19-64, however, we can pre-authorize or direct bill many third-party insurance plans on your behalf. If you would like us to do so, please print and sign the 2 benefit forms below and bring them to your appointment. Don’t forget to register on the portal above, and we look forward to seeing you!
